Proof of Birth is a one-time credential attached to the player's profile and verified by your Affiliate Parent.
NOTE: If an incorrect file is uploaded, the image is unclear, or the legal name does not match, the Proof of Birth will be rejected, and you'll receive an email to upload again.
From the Confirmation Email:
After purchasing your membership, you'll receive an email with your eligibility requirements. Click on the email from your club in your inbox.
Next to Proof of Birth, click Upload.
Log in to your account.
Follow the prompts to upload Proof of Birth.
From My SportsEngine Account:
After purchasing your membership, sign in to your account.
Notice of incomplete requirements appears on the Home page of your account.
At the top of the page, click Complete Requirement(s).
Locate Proof of Birth and click Upload.
Click Add File.
Select the file from your device.
Click Next.
Input your legal information exactly as displayed on the document, then click Next.
Review your information, then click Submit.
