Proof of Birth is a one-time credential attached to a player's profile and verified by your organization's affiliate parent, e.g., a National Governing Body or State.
NOTE: If an incorrect file is uploaded, the image is unclear, or the legal name does not match, the Proof of Birth will be rejected, and you'll receive an email to upload again.
From the Confirmation Email:
After purchasing your membership, you'll receive an email with your eligibility requirements. Click on the email from your club in your inbox.
Next to Proof of Birth, click Upload.
Log in to your account.
Follow the prompts to upload Proof of Birth.
From My SportsEngine Account:
Sign in to my.sportsengine.com and click the Memberships tab.
Locate the ineligible membership that requires Proof of Birth.
Click Upload and Add File.
Select the file from your device.
Click Next.
Input your legal information exactly as displayed on the document, then click Next.
Review your information, then click Submit.
