Below is information and steps to validate a USA Hockey membership ID. You can follow all of the steps in the article or click the links below to go to the specific section you are looking for:

NOTE:

  • The "USA Hockey Confirmation Number" (USAH #) must be on a separate page that is before the Contact Information.

  • Additionally, none of “Required Fields” can be removed from the registration session. Doing so will cause the registration session to error out. If there is a “required field” you wish to not include in the form, make that field “hidden” so it doesn't show up for general members.

The "USA Hockey Player Information" question group should be used if adding this functionality to a registration session. Once it's added, you will need to move the "USA Hockey Confirmation Number" field to its own page that appears before the page containing the remainder of the fields listed below.

Required Fields in Order for the Validation to Work (18 Fields)

  • USA Hockey Confirmation Number (separate page)

  • First Name

  • Last Name

  • Middle Initial

  • Birthdate

  • Gender

  • Address 1

  • Address 2

  • City

  • State/Province

  • Zip

  • Country

  • Citizen

  • Parent/Guardian First Name

  • Parent/Guardian Last Name

  • Phone Number 1

  • Phone Number 2

  • Email

How to Add a USA Hockey Membership ID Validating Question

NOTE:

  • It is best to copy a past registration with USA Hockey membership validation fields if you have one.

  • This article will outline how to add all validating fields to a new, blank registration.

  1. Navigate to the "Welcome Page" of the registration you are creating.

  2. Click into Edit Mode.

  3. On the left side of the screen, click Add New Form.

  4. In the Form Name field, input a name.

    • e.g., "Contact Information"

  5. Click Save Form.

  6. Click Add New Form again.

  7. In the Form Name field, input a name.

    • e.g., "USA Hockey Question"

  8. Click Save Form.

  9. Click, drag, and drop the "USA Hockey Question" form to the top of the pages underneath the "Who Are You Registering" form.

  10. Click on the "Contact Information" page you created.

  11. Click Add Question or Page Element.

  12. Under Add Questions, click Choose a Question Group.

  13. In the search bar, type in "USA Hockey Player Information".

  14. Click on the question group USA Hockey Player Information.

    • Those questions will now be added to your registration.

  15. Scroll down to the bottom of the page to find the USA Hockey Confirmation Number question.

  16. Click on the gray gear icon and choose Edit.

  17. Under The Question Format Is..., next to Confirmation Year Number(s), input the last digit of the second year that coincides with the correct season. For example:

    • 2 - 2021-2022

    • 3 - 2022-2023

    • 4 - 2023 - 2024

  18. Click Save Question.

  19. Click on the gray gear icon again, and choose Move.

  20. Next to Select Form, click the dropdown and choose the "USA Hockey Question" form created in the previous step.

  21. Click Move Page Element.
