NOTE: Downloadable AutoForms are optionally set up by the sports organization and may not be available for your registration.
Sign in and go to the Registrations tab of your dashboard.
Select the registration name that you need to print the AutoForm from.
At the top of the page, click on the hyperlinked name of the AutoForm PDF.
After the AutoForm downloadeds to your computer, print the form if necessary (optional).
