If your academy or club handles registrations manually, you will need to import your teams and rosters using the provided .CSV template.

If your academy or club handles registration using a third-party system, create a report with the following fields. Then, export the report and save it to your computer using a .CSV file type. If you require assistance with this step, please reach out to your third-party registration system’s support representatives.

NOTE: Fields with an "*" are required. During the import, you’ll be able to indicate the program, age group, and classification for each team.

Field

Definition

First Name*

The first name of the player/staff.

Last Name*

The last name of the player/staff.

Date of Birth*

Must use the MM/DD/YYYY format.

Gender*

Format Options:

  • “Male”, “Female”, or “Unspecified”

  • “M”, “F”, or “X”.

Email Address*

  • Must be in a valid format, i.e. [email protected].

  • For minors, use parent/guardian’s email address.

Team Name*

  • Will create the teams for roster submission, and cannot be left blank.

  • Must be unique to the academy/club.

  • Special characters are supported.

Team Role

Format Options:

  • Coach, Player, Registrar, Volunteer Coordinator, President, etc...

  • Cannot be left blank.

Middle Name

Format Options:

  • Full middle name (preferred).

  • Initials.

Phone

Parentheses and dashes are allowed but will be removed upon import.

Address 1

Must use the entire address in the following format:

  • Street Address, Province, Country, and Postal Code

Address 2

Must use the entire address in the following format:

  • Street Address, Province, Country, and Postal Code

City

Must use the entire address in the following format:

  • Street Address, Province, Country, and Postal Code

Province

Must use the entire address in the following format:

  • Street Address, Province, Country, and Postal Code

Postal Code

Must be a combination of 6 letters/numbers with a space in-between.

Country

Two letter code - i.e. CA
